+Submission Instructions
Submit
manuscripts according to the following directions:
1. Students
must submit with a faculty sponsor (see warrant statement on
Cover Page).
2. A
250 word structured abstract is required and should be provided
as a one page document. Components of a structured manuscript
differ by the type of presentation but should include:
Original Research: Objective, Design and Setting, Subjects,
Measurements, Results and Conclusions; Case Reports:
Objective, Background, Differential Diagnosis, Treatment,
Uniqueness, and Conclusions.
3. The
manuscript is limited to 4 pages and must be typed,
single spaced, with 1” margins on all sides including body of
manuscript, references, and figures.
The manuscript must be a complete description of the scholarly
work with specific or in-depth analyses of the topic. A blind
review process, ensuring anonymity for both authors and
reviewers, will be used for all paper submissions. The Review
and Selection Committee will review each manuscript for content,
clarity, and organization. Once accepted, authors may be asked
to revise and resubmit the paper according to suggestions from
the Review and Selection Committee and the Conference
Proceedings Editor(s). Manuscripts may be submitted by single or
multiple authors. Students are encouraged to submit papers based
on theses/dissertations or other scholarly work. Faculty and
students are encouraged to submit thesis/dissertation work
performed independently or scholarly research in collaboration
with colleagues.
4. Please
title the four (4) email file attachments as follows:
a. Cover
Page – Include your last name and the word "Cover" (e.g.,
Jones.Cover.doc).
i.
Include all author(s) identification/contact information and
institutional affiliation on the Cover Page. Indicate the
institution (including the city and state) where the research
was conducted on the same line following the name(s) of the
author(s). The manuscript should not contain any identifying
information as the Cover Page will be removed to facilitate the
blind review process.
ii.
Provide the Warrant Statement and signature line from the
faculty sponsor stating: "I, as faculty sponsor of this
submission, warrant that if this paper is accepted, will work
with the author(s) to address concerns of the CAHP Reviewers and
Proceedings Editors. We will submit a final (revised) MS Word
four page (single-spaced typed) paper for publication in the
Conference Proceedings by the deadline requested. Signed
_________________________________ (Faculty Sponsor).”
b. Abstract
– Include your last name and the word “Abstract” (e.g.,
Jones.Abstract.doc).
i.
Provide a structured
abstract of 250 words.
ii.
Include three (3) Key Words not in the Title that characterize
the focus of the paper
c. Original
Manuscript – Include your last name and the word
"Manuscript" (e.g., Jones.Manuscript.doc).
i. The
manuscript should be four (4) single spaced pages including
references and figures with one inch margins on all sides.
ii. Manuscripts
must be prepared following the American Medical Association
(AMA) Manual of Style 9th Edition (Williams &
Wilkins) and in compliance with the Uniform Requirements for
Manuscripts Submitted to Biomedical Journals found online
at:
http://www.icmje.org/.
d.
Blind Copy – Include BOTH your
abstract AND your manuscript and provide your last name and the
word "Blind" (e.g., Jones.Blind.doc) in the filename.
There should be no identifying
information concerning the author’s
name or institution in the Blind Copy.
5. Submit
the Cover Page, Abstract, Original Manuscript, and Blind Copy
via e-mail to
ebermanl@yahoo.com. The e-mail
attachments must be formatted as a MS Word document. The cover
email should include the presenter’s mailing address, city,
state, zip code, work phone, fax, and e-mail address.
Specifically, please title the subject line of the e-mail
message “CAHP 2006”.
6. AMA
Manual of Style 9th Edition guidelines must be adhered to
for format, organization, headings, reference citations,
grammar, and other issues. Submissions that do not follow these
guidelines will be returned to author(s). The Proceedings
Editor(s) may make necessary formatting and editing changes to
the manuscript for publication.
7. If
accepted for presentation, the original abstract will appear as
submitted in the Conference Proceedings. Edited manuscripts will
be published in the online Conference Proceedings.
